40 an hour 8 days ago 8 days ago 8 days ago The Care Coordinator EDWP (Elderly Disabled Waiverly Program) performs professional assessment and determination of a level of care and appropriateness for community-based services for Medicaid recipients or potential Medical Assistance for the Elderly and Disabled Waiver Program eligible clients.
They will function as a member of an interdisciplinary team including client's physician and serves large geographic areas which may include parts of one large county and/or many small counties which involve extensive travel.
May supervise Case Management team at direction of the Program Direction.
Requirements Education &
Experience:
Completion of a course of study in nursing with obtaining and maintaining a current RN/LPN License through the Georgia State Board of Nursing.
Must have at least two years of experience as a registered professional nurse in geriatric nursing, community health, long term care or chronic diseases of adults.
Certification:
CPR preferred.
Knowledge/Training:
Able to structure and process qualitative or quantitative data and draw insightful conclusions from it.
Identifies problems and opportunities.
Relates and compares data from different sources.
Determines root causes and subtle relationships.
Exhibits probe the mind and achieves penetrating insights.
Able to see and communicate the big picture in an inspiring way to provide seniors and persons with a functional and/or cognitive impairments stay in their homes and communities.
Computer Skills:
Must be proficient with general use and functions of computer technology including starting programs, using files and basic Windows Applications, using navigation buttons, and tool bars.
Transportation:
Must have reliable transportation.
